Job Summary: The Chronic Care Nurse Coordinator is responsible for coordinating multiple aspects of non-face-to-face patient care in a work-from-home setting. The Care Coordinator will conduct telephone assessments with patients and caregivers and will act as a liaison between the patient, primary care provider, and community resources.  The ideal candidate will be highly skilled at communication, organization, cultural intelligence, patient advocacy, and prioritization and will be tech-savvy, highly motivated, and a team player. • Maintain a dedicated patient panel.
• Review medical records for pertinent information.

• Perform non-face-to-face assessment of patients, including but not limited to assessment of medical and psychosocial needs, home safety, medication reconciliation, transportation and caregiver needs, care plan compliance, and activities of daily living.
